1. 1 - Original MOE (Ministry of Education) Paper ( for Saudi applicaut )
2. 2 - Medical Clearance
   1. A - Lab works (Blood Group, HBs Ag, HBs Ab, HCV Ab, HIV, Rubela Ab IgG, Measles Ab IgG, Mumps Ab IgG, Varicella Zoster Virus Ab IgG) and PPD or Mantoux test.
   1. B - Vaccines (Diptheria, Hepatitis B and Meningitis) (Rubela, Measles, Mumps and Varicella Zoster
3. 2 - Passport Copy
4. 3 - National ID Copy
5. 4 - Transcript of Records
6. 5 - Graduation Certificate Copy
7. 6 - Updated BLS Certificate Copy
8. 7 - Filled out Personal Data Sheet [ Download File ]
9. 8 - Photo
10. 9 - Consent Form (Non-Saudi) [ Download File ]
11. 10 - Confidentiality Agreement [ Download File ]
12. 11 - Payment of 24,000 SAR for 12 months rotation (if applicable)
